Since our last conversation on August 21st you had mentioned you were going to resend the documents for your dad, Bill Greene so we could submit the AR Choices application to get him the care he needs. I have the following documents: Social security award letter, picture of Humana card and title to the vehicle registered in his name. The only missing documents are updated bank statements for: June 2024 to August 2024. If you could get those over to me soon, we will submit the application within the next 24-48 hours. She stated she is a little frustrated because she has already sent this to NR when it was reqt'd and now she is 2 more months behind. MM informed her that we could submit her this week.
