Stopped by Billie’s apt today. I heard her inside talking, but she didn’t answer the door. She is very depressed and may not be ready. I will keep trying every time I go inside, but am changing status to not interested for her right now.

Stopped by to catch Billie. Left my card with a note to call me or have someone call me for her. If I don’t catch her next week, may put her on hold for awhile.

Sent referral to Elite HH. Spoke to Andrea. She said Billie was their patient over a year ago so they had all her info and will get an order from her pcp.

Met Billie and she is desperately needing help. She needed a change of address form so her mail can come to the right apt number. I went to post office and picked up form for her. She also needs home health for CHF, HTN, DM, and other chronic conditions.
