OPTUM EMAIL: MM SENT TO: KERRY, HEATHER, JOYCE, CLINT & NEVAEH: RE: Cindy Santiago I just spoke with Jazzman, the daughter of Cindy and she expressed that Cindy's husband is not happy as we have not been able to help them over the last 3 months to get care going. Cindy is not only a 618 but also ARC prospect. I attempted to get Jazzman to stay on board to schedule the OPTUM assessment, at minimum get her 64 hours a month and then I could send the list of supporting documents and details necessary to apply for the ARC program. Jazzman said her dad is looking in nursing facilities as he is needing Cindy to get the help she needs quicker than what this process is taking to get going. I empathised with the frustrations, Jazzman said she would talk to her dad about continuing on this path to get Cindy care and get back with me. Last week Cindy was scheduled for her initial assessment and I received a call from the OPTUM RN stating no one was answering the door or phone for her and I attempted to reach out to Jazzman without success. I will keep trying so we do not lose them to a facility.
