Followed back up no answer, Sent another follow up email. Good Morning, I am following up to do needing to find LTC placement for an individual that has filed for Social Security Benefits. I know when I work with Congress Womancks office they have a Social Security/Medicare Form that my patients can fill out and get a little help from his office on where they are at in the process so we can find LTC placement. Do you have such form?

I am trying to help him place him for the level of care that he is needing. also followed back up with Cm-SAM

Last week when I was in jonesboro I spoke with Cm in person. I told her i would send over a form for womacks office to see if we could get anything from his office on where he is. I emailed her the form she sent it back with it completed.

Tried to speak with SSA and they wouldnt give me any information. SSA
