We just spoke on the phone regarding the supporting documents needed in order to apply for the in-home personal care program through Medicaid, AR Choices. I have listed below the documents we will need before submitting the application: >Social Security Award Letter for Joan Edwards >Picture of front and back of all insurance cards for Joan Edwards >Last 3 months of bank statements (checking & savings) for Joan Edwards >Copy of Joan Edwards burial policy A few other questions I do want to clarify: >Does your mother, Joan, contribute to any bills in the home with you? If so, could you please write a statement that identifies how much she contributes. >Does Joan have a car registered in her name, even if she doesn't drive the vehicle? If so, we will need either the car registration or property tax statement. If she has sold, given away or traded a vehicle in the past 5 years we will need supporting documentation showing the vehicle is no longer in her possession.
