went by to pick up the bank statements. he tried to get them sent over into an email. after 3 tries I offered to come get them. I let him know i would get them over to our medicaid team.

Joie called me today that he has been trying to send the bank statements over and can't figure it out. Asked if someone could come over and pick them up. i told him i could. I would put him on my list them call him tomorrow to set a time for me to come

Spoke with Joie to see if he has gotten bank statements yet through 12/1. he said that they went monday and christine was supposed to send them to me. He said that he would make some calls and let me know.

I spoke with Joie this morning, he is going out collecting th bank statements and will get them emailed to me. I let him know I am here to help. I reminded him of the dates that they are looking for oct 1-dec1. he said ok.

Josh Gibson called me, he was there in the home with Joie. I told him what Joie told Nevaeh and he wants to start over. I told him we can. We went over the list. Josh helped him make the call to SSA, josh is helping him get the documents together. I told him i would help them then we will turn it over to my medicaid department.

Joie changed his mind and does not wish to proceed with this application at this time. He doesn't think he needs as much help and has HH to assist him with his medical needs explained the difference with home health and home care and he still declined at this time asked that we keep his information as he may change his mind in the future.
