Call from CC with daughter Ava. DOB 3/23/46. Inactive MMIS, lost 18-QMB on 11/12/23. Ava ready for ARC, she gets SSI (1200) burial (Kemper), bank.
DOB 3/23/46 and spoke to Dtr. Ava and it looks like discharge will be the 27th. She had Medicaid and then moved in with dtr. Ava 4 1/2 years ago. and the failed to change address and it messed up her Medicaid. She will need assistance with applying for Medicaid. I did a soft transfer to GR. CC
