07/24/2024: Any resolve from Lacy? If no ARC not ready
JC Sent the following email to Shannon Brewer, and Shanyce: Good afternoon, Shannon, I was tasked with attempting to get documents for Karon Sales to apply for ARC. I have been in contact with DR. Jones nurse Lacy who has been helping Karon gather the paperwork needed to apply for ARC I wanted to let you know last Lacy knew Karon was admitted to the swing bed program at OCH. Lacy states family is not willing to help gather the documents needed to apply for ARC. Without the financial paperwork like banking statements we will not be able to apply. Lacy is going to look into a solution but at this point I don't believe there is anything we will be able to do. I wanted to let you know. *Lcoate as well
F/U on ARC: JC received inbound phone call from Lacy at Dr. Jones office. Lacy states Karon requested assistance with applying g for LTC facility. But her family did not want het to make the move. Lacy states Karon was hospitalized and in the swing bed program at Ozarks Community Hospital. Lacy states she still has the paperwork she gathered before. Lacy states she will discuss w/ DR. Jones how to go about assisting Karon in getting her bank statements. Lacy will CB this afternoon. *WellSky
ARC CALL: 3rd attempt to contact to obtain paperwork from CL. has Lacy at OCH Clinic CB? She was assisting CL with getting documentation together. She is out of the office. Will return Monday 07/22/29. *WellSky file as well
