Stopped by visited with Micheal he is uncertain on wanting to return home due to the living conditions and the fear of not having his pain meds. Stated his wife will use his pain meds and leave him with none. Fu with Amber and informed him of his fear. APS has been called by Ivy Court do not know the outcome at this time. They are to be fixing the home in the mean time.
Spoke with Amber at Ivy court regarding Micheal they have decided to send him home at his request possibly 11/18 they where reaching out to see if we have a cg available to start reached out to bell she will look into it and fu with me 11/15

Stopped to visit Micheal he is at Ivy court he is not in a hurry to get release he is feared from threats to his son. AAA has already been called prior to him going to ivy court.
Assessment completed Janelle assisted on the Medicaid portion due to it being first Medicaid and making sure I filled it out properly
Spoke with Christian informed her we have spoken with the Medicaid office and they are in the process of sending over her office and we will set up an appointment to do an assessment as soon as we receive it
Spoke with Kristen daughter to Michael back on June 19. We are going to try to move forward. We have a few hours that we can possibly feel with this client. He was previously seen by Addis home care. Reach out to Lindsey. They do have an A&E waiver we put the request for change in waiting for Austin to come in told Kristen. I would reach back out to her as soon as we got authorization and get her set up.
