Prospect had AR-Choice and wanted to change providers; Found out prospect lost AR-Choice on 7/31/24; Assisted prospect with new ar-choice application; CK
Call from CK. LTD MMIS: 18-QMB (3360754001) Tier 1 DAAS 5/17/23. Lost ARC/MLTA 7/31/24. CK will check with DHS for ARC renewal docs & LOC info.
