She told a story of her mother passing away b/c "they" took her off her meds, her mother was in a nursing home and she passed away 4 days later. MM went on to explain the way a home care agency works, she is interested, has most of the docs already, she met a young lady recently that is a CNA, she would like her to be the CG. Called and spoke to Sharon went over demographics and documents for ARCH. She shakes really bad and can not take pictures or drive long distances so she will need a document PU. DOCUMENTS NEEDED FOR ARCH APPLICATION: SSI, insurance, bank, lease, vehicle, old life policy. CALL BACK IN 1 WEEK.
